The proper way to state this is “lock out/tag out”, it is a single act. There is no essential difference they are two expressions with the same meaning.
lock out tag out
Lock out is to isolate an energy system or some other system that could be harmful, when worked on when it is"live". Usually this involves some means of locking the isolating switches or valves, etc. In the isolated position.
Tag out is very much the same thing but usually involves a device where several people who work on the same system can use their personal padlocks on the locking tags so the system cannot be re-energised until all of the workers have removed their padlocks. This is to avoid a situation where somebody may energise the system with a colleague still working on it.
