How do you verify that Lockout/Tagout is effective?

Verification is the critical final step before beginning any maintenance work. The authorized employee must confirm that isolation and de-energization have been accomplished by:
Attempting to start and run the machine using its operating controls (the “try-start” procedure)
Verifying that locks and tags are properly attached to the correct energy-isolating devices
Checking gauges or using test equipment to confirm the absence of temperature, pressure, gases, vapors, or liquids
Ensuring all energy-isolating devices are in the “off” or safe position
If multiple authorized employees are working on the same equipment, each must independently verify isolation.
